29 CFR 1910.23(d)(2): Employer did not install fixed ladders with a minimum perpendicular distance of 7 inches (18 cm) (for elevator pit ladders, a minimum perpendicular distance of 4.5 inches, 11 cm); from the centerline of the steps or rungs, or grab bars, or both, to the nearest permanent object in back of the ladder; a) On or about 10/30/18 on the machine room roof; employees were exposed to a fall hazard when the third and fifth rungs on the east side of the fixed ladder cross over platform for the cooling tower water lines had a center line clearance of approximately 4 and 2 inches respectively, to the water lines in back of the ladder. ABATEMENT CERTIFICATION REQUIRED
