Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly. a) Facility: The employer failed to get the manufacturer's permission for the use of a jobmade man basket that was used on the forks of the forklifts, on or about 10/5/2018. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
