29 CFR 1910.119(i)(1): The employer did not perform a pre-startup safety review for new facilities and for modified facilities when the modification was significant enough to require a change in the process safety information. a) 626 Lynnway, Lynn, MA On or about August 27, 2018, a pre-startup safety review was not conducted when the ammonia refrigeration system was split, closing the production side of the facility, while the warehouse, chest, and cooler remained functional. In this switch, approximately 5700lbs of ammonia was removed from the system. These modifications were significant enough to require a change in process safety information. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
