29 CFR 1926.1053(a)(1)(i): Self supporting portable ladders were not capable of supporting, without failure, at least four times the maximum intended load: (a) Luis Ortiz Roofing, at 425 Wood Road, Snowmass Village, CO: On and preceding 8/28/18, the employer did not ensure that self supporting portable ladders were capable of supporting, without failure, at least four times the maximum intended load. Employees were re-roofing a four story condominium complex. Two approximately ten foot long job built ladders were used to access the fourth story roof from the third story. The side rails of each job built ladder were constructed of wood 2 x 4 pieces. The ladders did not have filler pieces between the wood 2 x 4 rungs of each job built ladder. At least one ladder was constructed by splicing 2 x 4 pieces as side rails. These conditions exposed employees to approximate ten foot fall hazards.
