29 CFR 1910.134(h)(1): Respirators were not cleaned and disinfected using the procedures in Appendix B-2 of 29 CFR 1910.134 or equivalent procedures recommended by the respirator manufacturer: The employer is failing to protect employees from hazardous associated with wearing a tight fitting respirator. The most recent occurrence was documented during the inspection conducted on July 30, 2018 on the jobsite located at 10004 NW 112th St, Malcolm, NE 68402. The employer has eight employees using tight fitting respirators when applying spray foam insulation. This process is conducted approximately three to five times per week. Respirators are not kept clean, sanitary, and in good working order.
