29 CFR 1926.20(b)(4): The employer did not ensure that the employee(s) required to operate equipment and machinery was qualified by training or experience. On or about June 19, 2018, and at times prior thereto, the employer was using all-terrain vehicles to transport employees off-road to the work location without ensuring that employees were thoroughly and effectively trained in the manufacturer's instructions of the Polaris Ranger ATV, exposing employees to the hazards of ejection.
