29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules and techniques to be utilized for the control of hazardous energy, including, but not limited to Items A D of this section: On or about May 21, 2018, employees were exposed to amputation hazards while troubleshooting the Blow Mold Machine. Employees used plastic tubing and tape to create a shim while the machine was running and energy control procedures were not developed for the task to control hazards associated with the hazardous energy.
