29 CFR 1910.147(c)(2)(ii): An energy isolating device that was capable of being locked out, did not utilize a lockout device, which rendered the machine or equipment inoperative: a) Polish Department - On or about April 9, 2018 and times prior to, the employer did not ensure that employees used effective energy isolating devices for the control of hazardous energy when performing maintenance activities on polish equipment such as but not limited to conveyors, thus exposing employees to caught-in hazards.
