29 CFR 1910.30(a)(3)(ii): Prior to any employee becoming exposed to a fall hazard, the employer did not ensure that all employees who use personal fall protection systems, or who is required to be trained, was adequately trained in the procedures to be followed to minimize those hazards. On or about April 4, 2018 and at times prior thereto, the employer did not train each employee exposed to fall hazards the procedures to be followed while working from the derrick board to minimize those fall hazards. This exposed employees to the hazard of falls.
