29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: a) Production Buildings: Affected employees were performing service and maintenance activities, such as clearing jams on the form sealer packaging machines (Ilapak and Jasa) and did not affix lockout devices onto machines to isolate the energy sources prior to performing the servicing and maintenance activities, observed on or about March 15, 2018. ABATEMENT CERTIFICATION REQUIRED FOR THIS VIOLATION
