29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. Lift Works, Inc.: The employer did not report an in-patient hospitalization, that was the result of a work-related incident to OSHA within twenty-four (24) hours for an injury that occurred on January 29, 2018. An employee suffered severe injuries on January 29, 2018 resulting in in-patient hospitalization, when the powered industrial platform, Nar Jib Boom, model #LWI2745, being unloaded from the truck bed, tipped over and fell on the employee.
