29 CFR 1910.303(b)(2): Installation and use. Listed or labeled equipment were not installed and used in accordance with any instructions included in the listing or labeling. (a) M/V Matanuska - Car Deck - On or about February 2nd, 2018 and at times prior thereto, employees were exposed to electrical hazards that would result in cardiac arrest, severe burns and death while connecting or disconnecting wiring in the circuit breakers of the temporary load centers. The employees were working in energized circuit breaker enclosures (GE TE100R Mod. 8 type 3R) with dead fronts that were intentionally removed and not re-installed per the listing and labeling from Underwriters Laboratories (UL).
