29 CFR 1915.55(f)(3): A hose carrying acetylene, oxygen, natural or manufactured fuel gas, or any gas or substances which may ignite or enter into combustion or be in any way harmful to employees, was not inspected at the beginning of each shift: (a) William E. Strait - On or about January 19, 2018 the employer did not ensure that employees using cutting torch in space below deck locker inspect the hose line supplying oxygen and propylene as required. A section of the hose was on the deck covered with snow and ice.
