29 CFR 1910.23(b)(9): Ladders were not inspected before the initial use in each work shift, or more frequently as necessary, to identify any visible defects that could cause injury: On or about November 7, 2017, and at times prior to: a) Maintenance department: The employer did not ensure ladders were inspected before they were used or at the beginning of each shift. Employees were exposed to fall hazards when working from the portable ladder with bent components. b) Hopper Line 1: The employer did not ensure that ladders were inspected before they were used or at the beginning of each shift. Employees were exposed to fall hazards while they worked from the portable ladder with bent components. c) Hopper Line 2: The employer did not ensure that ladders were inspected before they were used or at the beginning of each shift. Employees were exposed to fall hazards while they worked from the portable ladder with bent components.
