29 CFR 1926.1203(a): Before work begins, the employer did not ensure that a competent person identifies all confined spaces in which one or more of the employees are to enter and evaluates any permit required confined spaces: On or about November 15, 2017, at the above addressed jobsite, employees were performing construction type work activities, to include permit required confined space entry activities (elevator pit), wherein before the work began, the employer did not ensure that a Competent Person identified all confined spaces in which one or more of the employees were to enter, and did not evaluate any permit required confined spaces, and wherein an employee who was working in an elevator pit, was crushed by an elevator car and was subsequently seriously injured and hospitalized, and also resulted in exposing the other employees working at this job site to permit space physical hazards. NOTE: CERTIFICATION AND DOCUMENTATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
