29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces: a) Mercy Health Systems, d.b.a. St Vincents Medical Center, located at 2213 Cherry Street in Toledo, Ohio; On or about November 6, 2017 the employer did not ensure the workplace was evaluated to determine if any spaces were permit-required confined spaces. Employees entering (upper body) the San-I-Pak trash compactor, a decommissioned autoclave, to set pins were exposed to crush hazards from the energized ram of the compactor.
