29 CFR 1910.1030(d)(1): Universal precautions were not observed to prevent contact with blood or other potentially infectious materials: On or about October 26, 2017, the employer did not ensure that universal precautions were used as part of the process of removing skin, tissue, and blood from the Rockford Manufacturing Group wire feed coil machine (serial number 10816-1312) of the #20 cold former line and surrounding areas following an amputation incident occurring on October 26, 2017. The skin, tissue, and blood were not treated as if it were potentially infectious; employees did not wear gloves, proper decontamination was not performed, and regulated waste was not disposed of in appropriate containers
