29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. a. On or about October 3, 2017, employees were operating a CAT, Model EP6000, Serial FN408254, forklift equipped with a JibRig boom lifting attachment to handle granite slabs. The employer did not receive written approval from the equipment manufacturer for the use of the attachment and update the data plate on the forklift. b. On or about October 3, 2017, employees were operating a CAT, Model DP70, Serial T20C-607, forklift equipped with a shop made boom lifting attachment to handle granite slabs. The employer did not receive written approval from the equipment manufacturer for the use of the attachment and update the data plate on the forklift.
