29 CFR 1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two: a) Builder Services Group, Inc. dba Mato at 3355 County Road 27, Fort Lupton, Colorado 80621: On or before September 22, 2017, the exposing employer did not ensure that horizontal lifelines were designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two. Employees were installing insulation near the roof without adequate fall protection. A horizontal lifeline was set up for their use that was not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two. On September 22, 2017, an employee sustained serious injury when he fell approximately 24 feet to the concrete floor below. This condition exposed employees to fall hazards. Note: "29 CFR 1926.32(m) states: Qualified Person is defined as one who, by possession of a recognized degree, certificate, or professional standing, or who by extensive knowledge, training and experience, has successfully demonstrated his ability to solve or resolve problems relating to the subject matter, the work, or the project."
