29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: The employer is failing to protect employees from electrical shock hazards when performing window cleaning operations. This was most recently documented, on or about 7/12/2017, at 5431 SW 29th St, Topeka, Kansas 66604, where an unqualified employee was exposed to electrical shock hazards while utilizing a water fed extension pole within 10 feet of an unguarded energized overhead power line.
