29 CFR 1926.300(b)(2): Belts, gears, shafts, pulleys, sprockets, spindles, drums, fly wheels, chains, or other reciprocating, rotating or moving parts of equipment were not guarded when such parts were exposed to contact by employees or otherwise created a hazard: On or about June 22, 2017, at the jobsite located near 7727 Horatio Drive, Saint Louis, Missouri, the employer did not ensure the chain and sprocket were guarded on the IST Reel (Make: Ritec; Model: Motorized Cable Reel XXL; S/N: 81) mounted in the company truck (US DOT 171 4460; TVK 116). An employee sustained an amputation to the tip of the ring finger of the right hand while operating the reel.
