29 CFR 1910.178(l)(3)(i)(J): Powered industrial truck operators did not receive initial training on any vehicle inspection and maintenance that the operator would be required to perform: On or about June 29, 2017, employees were exposed to struck-by hazards due to the following: a. On June 29, 2017, the daily inspection sheet for Hyster #2 was checked indicating no problems when the problems with the head/tail lights and backup alarm/horn had not been corrected. b. Employees used Hyster #2 even when the daily inspection indicated problems with the head/tail lights, backup alarm/horn, and operating lever/controls during the weeks of June 19 and June 26. Hyster #3 was used even though the daily inspection sheet was checked indicating problems with head/tail lights (no head lights or brake lights) during the weeks of June 19 and June 26. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
