Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.119(f)(4): The employer did not develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel: Employees engaged in operating, inspecting, testing, and maintaining activities on the covered process equipment in the Tyson Prepared Foods facility located at 9 N Washington in South Hutchinson, Kansas were exposed to catastrophic release of ammonia which results in asphyxiation, explosion and flash fire leading to death and injury to persons in and near the facility in that the Employer did not follow established procedures and control entrance into the facility by support personnel employees.
