On or about March 10, 2017, employee(s) did not operate energy isolating devices so as to isolate energy sources during serving and/or maintenance activities on BOBBIO-CNC-1400 in the F-60 department. The list of these activities includes, but is not limited to, maintaining set up and replacing an arbor.
