29 CFR 1910.184(e)(1): Alloy steel chain slings did not have permanently affixed durable identification stating size, grade, rated capacity, and reach: (a) BOS Solutions, @ Anadarko Rig X22, 2000 CR 23, Fort Lupton, CO 80621: On or before November 28, 2016, as part of injury/accident prevention responsibilities under its safety program, the exposing employer did not ensure that the steel chain sling used for lifting had permanently affixed durable identification stating size, grade, rated capacity, and reach. The short steel chain was used to lift a shale bin onto a ground mat, when the chain snapped. This condition exposed employees to the hazard of being struck-by the chain and the falling load.
