Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about January 27, 2017 employees in the Custom Machine Shop performed servicing and maintenance tasks when performing setups on the following Horizontal Milling Machines: a. The Mori Seiki #732 b. The Okuma #738 c. The Toyota #734 Employees were not required to control hazardous energy sources in the following sequence: 1. Turn off and shut down the machines. 2. Physically locate and operate energy isolating devices in such a manner as to isolate the machines or equipment from the energy source(s). 3. Affix lockout or tagout devices to each energy isolating device. 4. Verify that isolation and de-energization of the machines or equipment have been accomplished. Employees were exposed to amputation and laceration hazards from the unexpected start up and/or the release of hazardous energy. Alternative protection, such a machine guarding, was not adequate. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1904.19(d).
