29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: a) Jobsite/Basement access - A floor hole used to access a basement was not protected by a guardrail system or other means of fall protection exposing employees to fall hazards of approximately 12 feet. b)Jobsite/Next to stairwell - A caution tape was used around a floor hole in lieu of guardrails or other means of fall protection exposing employees to fall hazards of approximately 12 feet. Condition was observed on 27 DEC 2016.
