Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(a)(2)(ii)(B): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee (s) was required to place any part of his or her body into area on a machine or piece of equipment where work was actually performed upon the material being processed (point of operation) or where an associated danger zone existed during a machine operating cycle: a) Woodland Manufacturing - SE Chang Machine: Employees were required to place their hands into the point of operation of the SE Chang Corrugated Carton Flexo Printing Machine. An employee suffered an amputation while cleaning the roll of the machine. Occurred on or about 12/16/2016. b) Woodland Manufacturing - EMBA Box Machine: An employee suffered crushing injuries when he was placing part of his body on or near pinch points while connecting the glue feeder to the EMBA machine # 634002. Another employee was performing set up operations on the EMBA Machine at the same time, which caused the machine to cycle while the other employee was exposed to pinch points. Occurred on 01/16/2017.
