29 CFR 1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck: Machine Shop, Chalmers and Kubeck North, 24-34 Elise Street, Westfield, Massachusetts; On and before November 8, 2016 the employer permitted employees to drive powered industrial trucks and the employees had not completed the required training prior to operating the powered industrial trucks.
