29 CFR 1910.147(e)(2)(ii): After lockout or tagout devices were removed and before machines or equipment is started, affected employees were not notified that the lockout or tagout device(s) have been removed: A) Terex Utilities, Inc. DBA Terex Services, conducting work at 1300 Marion-Agosta Road, Marion, OH 43302: On or about September 16, 2016, the employer did not ensure that the work area was checked and the employee had been safely positioned or removed from the hazardous area after the employee completed repairs of the Whiting 30-Ton overhead crane (Serial # 12171), before the energy was restores to the crane, and the crane was moved.
