29 CFR 1910.119(l)(1): The employer did not establish and implement written procedures to manage changes to process chemicals, technology, equipment, and procedures; and, changes to facilities that affect a covered process. On September 7, 2016 and at times prior thereto and at subsequent times, the employer at the Blanchard Refining Galveston Bay Refinery did not implement written procedures to manage changes to the discharge piping of the P1008B Depropanizer Reflux Pump when the spectacle blind at the flange that joins the pump to the discharge piping was changed out and the replacement blind was not an in-kind replacement. b) The spectacle blind at the discharge of the P-1008B Depropanizer Reflux Pump was changed to a spectacle blind with an integrated bleed feature without implementing the GBR procedure for managing changes to processes.
