29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a) Warehouse - Employees operating forklifts including but not limited to a Hyster R30ES and a Landoll B40 Bendi, were not evaluated in the last three years, on or about 9/6/16. Note: In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
