29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a. Metal Conversions, Ltd. - Mansfield, Ohio: On or about August 25, 2016, employees operated powered industrial vehicles to lift and maneuver materials such as, but not limited to, boxes and bales of metal. The employer did not ensure that employees successfully completed a forklift operator training program containing elements in 29 CFR 1910.178(l) before being permitted to operate the forklift.
