29 CFR 1910.269(l)(4)(i): The employer did not ensure that when an employee uses rubber insulating gloves as insulation from energized parts (under paragraph (l)(3)(iii)(A) of this section), the employee shall also use rubber insulating sleeves: On or about July 23, 2016, in the yard of a residential home where an apprentice was exposed to electrical hazards while connecting drop lines into an underground residential distribution (URD) transformer without using insulating sleeves or other means of insulation from the energized parts.
