29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: a) On or about June 29, 2016, and at times prior thereto; the employer did not ensure that employees performing servicing, maintenance and/or mounting/installing Die cutting blades were trained. Employees were exposed to struck-by, crushed-by moving parts, and/or amputation while performing work on Die cutting machines.
