29 CFR 1910.119(e)(3)(vii): The process hazard analysis (PHA) did not address a qualitative evaluation of a range of the possible safety and health effects of failure of controls on employees in the workplace. a) In the facility at 238 Nonotuck Street Florence, MA: On or about August 15, 2016, the PHA did not address a qualitative evaluation of a range of possible health and safety hazards of Formaldehyde exposure due to a failure of controls during Formaldehyde tanker truck bulk deliveries. b) In the facility at 238 Nonotuck Street Florence, MA: On or about August 15, 2016, the PHA did not address a qualitative evaluation of a range of possible health and safety hazards of Formaldehyde exposure due to thermocouple failure and misidentification of the failure due to programmable logic controller (PLC) logic issues. c) In the facility at 238 Nonotuck Street Florence, MA: On or about August 15, 2016, the PHA did not address a qualitative evaluation of a range of possible health and safety hazards of Formaldehyde exposure due to a high temperature condition of the reactor prior to the introduction of melamine.
