29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. Instance a) This violation was most recently observed on or about June 21, 2016 and times prior thereto and times thereafter, in the Refrigeration Process at American Cold Storage (ACS) located in Tulsa, Oklahoma when the employer did not ensure the engine room was equipped with an ammonia detector that actuates an alarm and mechanical ventilation at a value not greater than the corresponding TLV-TWA. The alarm shall annunciate visual and audible alarms inside the refrigerating engine room and outside each entrance to the machine room as stipulated in RAGAGEP such as ANSI/ASHRAE Standard 15-2013. Instance b) This violation was most recently observed on or about June 21, 2016 and times prior thereto and times thereafter, in the engine room when the employer failed to ensure that the machine room back exit was equipped with approved, tight-fitting, self-closing doors as stipulated by IIAR-2, section 4.4.8.
