29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about March 30, 2016 and at time prior to that date, for the employees exposed to stuck-by hazards while operating a shop built 75 ton hydraulic press without barrier guarding in the welding and fabrication room at 104 First Street Southwest, Cooperstown, North Dakota. (b) On or about March 30, 2016 and at time prior to that date, for the employees exposed to stuck-by hazards while operating a shop built 175 ton hydraulic press without barrier guarding in the welding and fabrication room at 104 First Street Southwest, Cooperstown, North Dakota. (c) On or about March 30, 2016 and at time prior to that date, for the employees exposed to amputation hazards while operating a Cincinnati Shear, Model No. 1810, Serial Number 37348 with two missing hydraulic holddown clamp guards in the fabrication room at 104 First Street Southwest, Cooperstown, North Dakota.
