Text For Citation: 01 Item/Group: 002A Hazard:
29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department or other group: a) Plant, Airlay Building - On or about 03/08/16 and at times prior thereto employees were assigned to clean the airlay equipment while only one person locked a main panel in the control room. Workers were not protected from the sudden start of machinery and potential errors during shift changes. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.
