29 CFR 1910.23(a)(8)(ii): Every floor hole into which persons could accidentally walk were not guarded by a floor hole cover of standard strength and construction: a) At 179 Rushing Road in Scott City, MO 63780 (liquid fertilizer loading area) - On or about 3/16/2016, a floor hole measuring approximately 6 inches in diameter, 3 inches at smallest point and 4-5 inches in depth, which employees could accidently walk was not guarded by a floor hole cover of standard strength and construction.
