29 CFR 1910.26(c)(2)(iv): Ladders were not maintained in a good usable condition at all times. On or about March 9, 2016, and times prior thereto, the employer did not ensure A-frame ladders used daily by employees were kept in a usable condition. This exposed employees to fall hazards.
