29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) Facility, register area: Temporary power taps were not used as approved by their listing and labeling. The temporary power taps were piggy backed to supply electrical power to the different electrical devices located around the register. Temporary Power tap model AS-912 was plugged into the Temporary Power tap, model # 63010 which was plugged into the temporary power tap, model # A-1193-004-ASSLV which was plugged into the 4 inch by 4 inch metallic outlet box, on or about 2/24/16. NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM, FAILURE TO DO SO WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 AS PER 29 CFR 1903.19.
