29 CFR 1910.252(a)(2)(iv): Cutting or welding was permitted in areas before the areas were inspected by the individual responsible for authorizing cutting and welding operations: On or about January 25, 2016, the outside area at the back of the shop was not inspected prior to allowing employees to perform oxygen-acetylene torch cutting tasks. Employee performed cutting tasks in proximity to an oil tank that contained a residual amount of a flammable/combustible liquid mixture. Sparks from the cutting tasks ignited with the vapors from the tank and exposed employees to a fire and explosion hazard.
