29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: (a) 2 16th Street, Wheeling; First floor: On or about January 15, 2016, employees cut out a 8'x20' foot floor hole for a new stairway installation. Employees were exposed to a 16 foot fall hazard from the unprotected floor hole. (b) 2 16th Street, Wheeling; Second Floor: On or about January 15, 2016, employees cut out a 8'x20' foot floor hole for a new stairway installation. Employees were exposed to a 11 to 27 foot fall hazard from the unprotected floor hole. (c) 2 16th Street, Wheeling; Third Floor: On or about January 15, 2016, employees cut out a 8'x20' foot floor hole for a new stairway installation. Employees were exposed to a 11 to 38 foot fall hazard from the unprotected floor hole.
