29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: a) Sign Source USA, Inc. - Lima, Ohio: On or about December 15, 2015, the employer failed to provide an employee who was required to wear tight-fitting half mask respirator while painting with a medical evaluation to determine if the employee was fit to wear the respirator. b) Sign Source USA, Inc. - Lima, Ohio: On or about January 14, 2016, the employer failed to provide an employee who was voluntarily wearing a tight-fitting half mask respirator while grinding in the Heavy Fabrication Shop with a medical evaluation to determine if the employee was fit to wear the respirator. Sign Source USA, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection #110898, Citation 01, Item 7 and was affirmed as final order on April 25, 2012, with respect to a workplace located at 1700 S. Dixie Hwy, Lima, Ohio 45804.
