29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: (a) Fill room mixing area - On or about 11/19/15, a mixer employee's eyes and face were exposed to liquid splashes from chemicals such as, but not limited to, 93% sulfuric acid (pH of 0.3) and 45% potassium hydroxide (pH of 12 - 14) when transferring them from a bucket to a stainless steel mixing tank. The employer did not ensure that the employees wore appropriate chemical splash goggles and an appropriate face shield. ABATEMENT DOCUMENTATION REQUIRED
