29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) Upper Level - A supervisor was working on the adult adventure challenge seventeen (17) feet three (3) inches above the ground without the benefit of a fall protection system, thereby exposing him to a fall to the ground below, on or about October 23, 2015. (b) Lower Level - An employee was working on the adult adventure challenge six (6) feet Eight (8) inches above the ground without the benefit of a fall protection system, the employee fell from the platform and suffered fatal injuries when he struck the rocks below, on or about October 23, 2015. No abatement certification or documentation required.
