29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork while the platform was occupied. a) Jobsite: Building 5 - A job-made platform (wooden box) was not attached to the forks of the Caterpillar Telehandler while employees occupied the platform to perform gutter installation activities. This condition exposed employees to collapse hazards. Noted on or about 10/5/15.
