29 CFR 1926.959(d)(2): A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment to provide timely warnings before the minimum approach distance required by paragraph (d)(1) of this section was reached. a) 2998 Glenwood Drive, Camp Hill, Pa - The employer did not assign an observer to ensure that the approach distance to the energized 7.2kV transformer was maintained to minimize the possibility of accidental employee contact, thereby exposing employees to electrical hazards on or about September 28, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
