29 CFR 1926.960(c)(1)(i): The employer did not establish minimum approach distances (MAD) no less than the distances computed by Table V-2 for ac systems: On or about September 9, 2015 the employer did not establish a minimum approach distance (MAD) for energized work on or near 13Kv (phase to phase) power lines that was equal to or greater than the 2.13 feet required by Table V-2. Or, in the alternative: 29 CFR 1910.269(l)(3)(i): The employer did not establish minimum approach distances not less than the distances computed by Table R-3 for ac systems: a) On or about September 9, 2015 the employer did not establish a minimum approach distance (MAD) for energized work on or near 13Kv (phase to phase) power lines that was equal to or greater than the 2.13 feet required by Table R-3.
